Pragmatic Free Trial Meta Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, which allows for multiple and varied meta-epidemiological studies that evaluate the effect of treatment on trials that have different levels of pragmatism as well as other design features. Background Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the usage of the term "pragmatic" is not uniform and its definition as well as assessment requires clarification. Pragmatic trials are intended to inform clinical practices and policy choices, rather than prove a physiological or clinical hypothesis. A pragmatic study should strive to be as close as possible to the real-world clinical practice, including recruiting participants, setting, design, delivery and implementation of interventions, determination and analysis results, as well as primary analysis. This is a significant difference between explanation-based trials, as defined by Schwartz & Lellouch1 which are designed to confirm the hypothesis in a more thorough way. Truely pragmatic trials should not conceal participants or clinicians. This can result in bias in the estimations of treatment effects. In addition to these features pragmatic trials should reduce the trial's procedures and requirements for data collection to reduce costs. In the end the aim of pragmatic trials is to make their findings as applicable to current clinical practice as is possible. This can be achieved by ensuring that their primary analysis is based on the intention-to treat approach (as defined in CONSORT extensions). Despite these guidelines, many RCTs with features that challenge the concept of pragmatism have been mislabeled as pragmatic and published in journals of all kinds. This could lead to misleading claims of pragmatism and the use of the term needs to be standardized. The development of the PRECIS-2 tool, which offers an objective standard for assessing practical features, is a good first step. Methods In a practical study the aim is to inform clinical or policy decisions by demonstrating how an intervention can be integrated into routine care in real-world situations. This differs from explanation trials, which test hypotheses about the causal-effect relationship in idealized situations. Consequently, pragmatic trials may be less reliable than explanatory trials and might be more susceptible to bias in their design, conduct and analysis. Despite their limitations, pragmatic studies can be a valuable source of information to make decisions in the context of healthcare. The PRECIS-2 tool evaluates the level of pragmatism that is present in an RCT by assessing it across 9 domains, ranging from 1 (very explicit) to 5 (very pragmatic). In this study the domains of recruitment, organisation, flexibility in delivery, flexibility in adherence, and follow-up scored high. However, the principal outcome and method of missing data was scored below the pragmatic limit. This suggests that it is possible to design a trial that has good pragmatic features without harming the quality of the outcomes. It is, however, difficult to assess how pragmatic a particular trial really is because pragmaticity is not a definite attribute; some aspects of a trial can be more pragmatic than others. The pragmatism of a trial can be affected by modifications to the protocol or the logistics during the trial. Koppenaal and colleagues discovered that 36% of 89 pragmatic studies were placebo-controlled, or conducted prior to the licensing. They also found that the majority were single-center. They are not in line with the standard practice, and can only be referred to as pragmatic if the sponsors agree that such trials aren't blinded. Furthermore, a common feature of pragmatic trials is that the researchers attempt to make their findings more valuable by studying subgroups of the sample. Additionally, studies that are pragmatic can present challenges in the gathering and interpretation of safety data. This is because adverse events are typically reported by participants themselves and are prone to reporting delays, inaccuracies, or coding variations. It is essential to improve the quality and accuracy of the outcomes in these trials. Results Although the definition of pragmatism does not require that all trials be 100 100% pragmatic, there are some advantages to including pragmatic components in clinical trials. These include: Incorporating routine patients, the trial results can be translated more quickly into clinical practice. However, pragmatic studies can also have drawbacks. The right type of heterogeneity for instance could help a study extend its findings to different patients or settings. However the wrong kind of heterogeneity can decrease the sensitivity of the test and thus lessen the power of a trial to detect small treatment effects. A variety of studies have attempted to classify pragmatic trials with various definitions and scoring systems. Schwartz and Lellouch1 developed an approach to distinguish between explanation-based trials that support a physiological or clinical hypothesis as well as pragmatic trials that aid in the selection of appropriate therapies in real-world clinical practice. It is crucial to keep in mind that a pragmatic study should not mean a low-quality trial. In fact, there is increasing numbers of clinical trials that use the word 'pragmatic,' either in their abstracts or titles (as defined by MEDLINE, but that is not precise nor sensitive). These terms may signal that there is a greater awareness of pragmatism within titles and abstracts, but it isn't clear whether this is evident in the content. Conclusions In recent years, pragmatic trials have been becoming more popular in research as the importance of real-world evidence is becoming increasingly acknowledged. They are clinical trials randomized that evaluate real-world alternatives to care instead of experimental treatments in development, they include patients that more closely mirror the patients who receive routine medical care, they utilize comparators which exist in routine practice (e.g. existing medications), and they depend on the self-reporting of participants about outcomes. This method can help overcome the limitations of observational research, like the biases that are associated with the reliance on volunteers, and the lack of codes that vary in national registers. Other advantages of pragmatic trials include the possibility of using existing data sources, and a greater chance of detecting meaningful changes than traditional trials. However, these tests could be prone to limitations that undermine their reliability and generalizability. related in certain trials may be lower than expected due to the health-promoting effect, financial incentives, or competition from other research studies. The requirement to recruit participants in a timely manner also limits the sample size and impact of many pragmatic trials. Practical trials aren't always equipped with controls to ensure that any observed differences aren't caused by biases in the trial. The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described as pragmatism. The PRECIS-2 tool was used to evaluate pragmatism. It includes areas such as eligibility criteria and flexibility in recruitment, adherence to intervention, and follow-up. They found 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains. Trials with a high pragmatism score tend to have higher eligibility criteria than traditional RCTs which have very specific criteria that are not likely to be found in clinical practice, and they include populations from a wide variety of hospitals.
