Pragmatic Free Trial Meta Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, permitting multiple and varied meta-epidemiological research studies to evaluate the effect of treatment on trials that have different levels of pragmatism, as well as other design features. Background Pragmatic studies provide real-world evidence that can be used to make clinical decisions. The term "pragmatic" however, is a word that is often used in contradiction and its definition and evaluation need further clarification. Pragmatic trials should also seek to enroll patients from a wide range of health care settings to ensure that the results can be applied to the real world. Furthermore, pragmatic trials should focus on outcomes that are crucial for patients, such as quality of life or functional recovery. This is particularly relevant in trials that involve surgical procedures that are invasive or have potential for serious adverse events. The CRASH trial29, for instance focused on the functional outcome to evaluate a two-page case report with an electronic system for the monitoring of patients admitted to hospitals with chronic heart failure. Similarly, the catheter trial28 used urinary tract infections caused by catheters as the primary outcome. In addition to these characteristics the pragmatic trial should also reduce the procedures for conducting trials and requirements for data collection to reduce costs. Additionally the aim of pragmatic trials is to make their results as relevant to real-world clinical practice as is possible. This can be accomplished by ensuring that their analysis is based on an intention-to treat method (as defined in CONSORT extensions). Many RCTs that do not meet the criteria for pragmatism but have features that are in opposition to pragmatism, have been published in journals of varying kinds and incorrectly labeled pragmatic. This can lead to false claims about pragmatism, and the term's use should be made more uniform. The creation of a PRECIS-2 tool that offers an objective, standardized evaluation of pragmatic aspects is the first step. Methods In a pragmatic study the goal is to inform policy or clinical decisions by demonstrating how an intervention would be integrated into everyday routine care. Schwartz and Lellouch1 have developed a framework that can discern between explanation-based studies that confirm a physiological hypothesis or clinical hypothesis, and pragmatic studies that inform the selection of appropriate therapies in clinical practice. The framework was comprised of nine domains, each scoring on a scale ranging from 1-5, with 1 indicating more explanatory and 5 indicating more practical. The overall score was lower for systematic reviews that were pragmatic when the domains on organisation, flexible delivery and follow-up were merged. It is important to understand that a pragmatic trial doesn't necessarily mean a low quality trial, and there is an increasing number of clinical trials (as defined by MEDLINE search, however it is neither specific or sensitive) that use the term "pragmatic" in their title or abstract. The use of these words in abstracts and titles may suggest a greater awareness of the importance of pragmatism, however, it is not clear if this is manifested in the content of the articles. Conclusions As the value of evidence from the real world becomes more popular and pragmatic trials have gained traction in research. They are clinical trials randomized that evaluate real-world alternatives to care instead of experimental treatments in development, they include patients that are more similar to the patients who receive routine care, they use comparators that are used in routine practice (e.g., existing drugs), and they rely on participant self-report of outcomes. This approach could help overcome the limitations of observational studies, such as the biases associated with reliance on volunteers, and the limited accessibility and coding flexibility in national registry systems. Pragmatic trials also have advantages, like the ability to use existing data sources and a greater chance of detecting significant differences than traditional trials. However, they may still have limitations which undermine their validity and generalizability. The participation rates in certain trials may be lower than expected because of the healthy-volunteering effect, financial incentives, or competition from other research studies. The need to recruit individuals in a timely fashion also limits the sample size and the impact of many practical trials. Additionally, some pragmatic trials do not have controls to ensure that the observed differences are not due to biases in the conduct of trials. ?? of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and that were published up to 2022. The PRECIS-2 tool was used to evaluate pragmatism. It includes areas such as eligibility criteria and flexibility in recruitment and adherence to intervention and follow-up. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or higher) in at least one of these domains. Trials that have a high pragmatism score tend to have higher eligibility criteria than traditional RCTs which have very specific criteria that are not likely to be used in clinical practice, and they comprise patients from a wide range of hospitals. These characteristics, according to the authors, may make pragmatic trials more useful and relevant to everyday practice. However they do not guarantee that a trial will be free of bias.
